Subject:          Blanche Landis - Request for Leave Donations



Blanche Landis has suffered a catastrophic illness/injury and is unable to
return to work for some time.  She has exhausted all of her paid leaves and
is in need of donated sick leave and/or vacation credits.



The University allows employees who accrue sick leave or vacation credits
to donate leave credits to help another University employee in his/her time
of need.



Employees in Units 1 (Physicians), 8 (Public Safety), & 11 (Teaching
Associates only) may donate 1 to 16 hours of leave credits per fiscal
year.   Employees in Units 2, 5, 7 & 9 (CSUEU), 3 (Faculty), 4 (Academic
Support), 6 (Skilled Trades) along with MPP (M80), Executive (M98),
Confidential (C99) and Excluded (E99) employees may donate 1 to 40 hours of
leave credits per fiscal year.  You may donate either sick leave or
vacation (or both) in one-hour increments.



Your consideration in this matter is appreciated.  If you desire to donate
leave credits on behalf of your fellow employee, you may do so by
completing the attached form and returning it to the Payroll Office
(MC-1625).



Program guidelines provide that only sufficient leave credits be used as
needed for each pay period.  As your donated credits are used for the
benefit of this employee, you will receive a confirming memo from the
Payroll Office.
