Attention HR Liaisons:

 

Please forward the attached notification to faculty and staff in your
department regarding the CalPERS Dependent Eligibility Verification project.
The deadline to submit required documentation is September 12, 2013. Failure
to submit complete documentation for enrolled dependents by the deadline
will result in a removal of the dependents from coverage.

 

Employees are required to call HMS, the third party administrator, directly
at 877-796-9628 for questions or additional information.
