[Staff] Important! Nur Yazdani Survey - An UPDATED version has been sent!

Hi Everyone,

I have been notified by several faculty and staff that they are not able to
take the survey for Dr. Nur Yazdani that I sent out earlier today.  Please
DO NOT enter any of your personal information.

I just sent out an UPDATED survey for Dr. Yazdani. In the original survey
that I sent through Survey Monkey, I used the faculty and staff listserve
e-mail address which only allowed one faculty and one staff member to
complete the survey, due to Survey Monkey assigning a unique URL address
for each e-mail address.

On the UPDATED survey that you will be receiving shortly, I entered each of
your e-mail address individually, so that each address will have its' own
unique URL address and the survey should immediately start when you click
on the link that is e-mailed to you.

If you have any further issues with the UPDATED survey, please let me know,
but I'm hoping that everything should be working with the individual e-mail
addresses entered.
