[Staff] December Attendance Summaries - Due: Tue 1/5

Hi Everyone,

I have been informed by Payroll that our Attendance Summary sheets need to
reflect the correct dates of the SDSU pay periods.  For December 2015, the
pay period is from *12/2-12/31*, please use the attached December
Attendance Summary, which has the correct dates listed to report your time.

For Monday, 12/21, we were given a half day (4 hours)  of informal time
from Governor Brown and a half day (4 hours) of administrative leave from
President Hirshman.  Please fill in these hours accordingly in the informal
time and administrative leave sections if you took the time for 12/21.   If
you had to work due to operational demands, and those who were scheduled to
take vacation, sick or compensatory time off my be granted an equivalent
amount of informal time off to be taken by June 30, 2016.

*December Attendance Summaries are due by end of day Tuesday, January 5,
2016. *

*Please note, per Payroll*:  For 12-month appointment employees (i.e., Dept
Chairs, Asst/Assoc Dean, MPP and Staff),  *even if you do not have time to
report, you have to complete an Attendance Summary form and mark the NO
USAGE box located at the top of the form.*

*Faculty* only need to complete an attendance summary for *sick leave.*

Please drop off attendance summaries in the Dean's mailbox, or you can drop
them in my office in E-203B.
