[Faculty] Urgent: Critical/Essential Research Functions

Dear Colleagues,

The administration refined the template I sent you last night; they are
gathering more data. Please complete the attached form as soon as possible.
I strongly recommend that you fill-in the entire field since the data will
be used to grant or deny access to labs.

The new due date for the completed form is *12:00pm Friday, March 27, 2020*.
